Sculptra nodules: why do they form, and how should you massage?

Sculptra nodules are small grains or lumps felt under the skin, formed when injected poly-L-lactic acid (PLLA) particles clump in one spot or collagen gathers around them. Clumping felt in the first few days after treatment means the product hasn't spread evenly yet, so it is smoothed out with massage, while nodules that newly harden after several weeks should be examined.
Why do Sculptra nodules form?
Sculptra does not fill volume immediately like a filler; instead, the body reacts to PLLA particles and produces collagen around them. If many particles gather in one spot, the collagen reaction grows only in that spot, which can result in a palpable nodule.
Nodules tend to form when the product is diluted with too little fluid and is highly concentrated, when a lot is injected in one spot, or in areas with thin skin and lots of facial muscle movement, such as around the eyes and lips. Nodules can also appear late, from several weeks to several months after treatment.
How to massage to reduce Sculptra nodules
At our Suwon clinic, we advise gently massaging 5 times a day, about 5 minutes each time, for at least 5 days after Sculptra treatment.
- With clean fingertips, gently rub the injected areas in circular motions.
- Applying moisturizing cream first makes the skin feel less tight and helps your fingers glide.
- Don't press hard enough to hurt or pinch.
- Remove the DuoDERM dressing applied to the treated area after 3 days.
For 2 to 3 weeks, avoid positions that press on the treated area for a long time, sleeping face down, facial meridian massage, and ultrasound or radiofrequency treatments. For 3 to 7 days, also refrain from heavy drinking, smoking, saunas, and vigorous exercise.
What should I do about Sculptra nodules I can feel?
Clumping, a foreign-body sensation, and swelling felt within 1 to 2 weeks after treatment mostly ease. If grains can still be felt after several weeks or are getting bigger, or if a nodule visibly protrudes, after an examination we decide on methods such as injection treatment depending on its size and location. Unlike hyaluronic acid filler, Sculptra cannot be dissolved with an enzyme, so it is best to have it checked early.
If the injection site becomes red and swollen with warmth and pain, or the skin turns white or purplish, please contact the clinic right away. If you are prone to keloids, have an autoimmune disease, or are pregnant or breastfeeding, please let us know before treatment.

FAQ
Q. Do Sculptra nodules go away on their own?
A. Clumping felt in the first few days after treatment often fades within 1 to 2 weeks along with massage. Nodules that newly harden after several weeks can take a long time to shrink on their own, so have their size and location examined and decide on a treatment method.
Q. If I skip the Sculptra massage, will nodules form?
A. Massage is the process of spreading particles evenly so they don't gather in one spot, so skipping it may increase the chance of clumping. If you missed a day or two, keep to the schedule from then on, and if you feel any grains, please let us know.
Q. How many sessions is Sculptra divided into?
A. Injecting a large amount at once increases the risk of clumping, so it is usually given in 3 or more sessions at 1-month intervals to build up collagen changes. Changes appear gradually over 4 to 6 weeks, and the number of sessions depends on the degree of hollowness.
